Patient – Citizen Forms
- Application for the Issuance of Certificates and Statements (medical opinion, copy of medical history, referral file, copies of hospitalization records, etc.)
- Private Correspondence Form.
- Complaints and Reports Form.
Responsibilities
- Responsible for receiving applications from patients.
- Provides clarification regarding any questions about completing the application form.
- Verifies the supporting documents (identity card, family status certificate, public prosecutor's order, etc.).
- Receives and delivers, or sends by regular mail, responses to patients’ requests.
Clarifications
- 1 The documents referred to in the Application are issued following the submission of a relevant written and officially registered application by the interested party or, in the case of a minor, by their legal guardian.
- 2 Every application must state a specific reason. The phrase “for any lawful use” should not be used.
- 3 An application may be submitted by the interested party if they are an adult, by a parent on behalf of a minor child, by the Public Prosecutor’s Office, or by a third party with a legitimate legal interest.
- 4 The application may be completed and submitted in person, by fax, by email, through an authorized third party, or via a Citizens’ Service Centre (KEP). If the application is signed by the child’s father and sent by FAX or ELECTRONICALLY, a copy of his identity card must also be provided. If the application is completed by the mother, a family status certificate or another document proving that she is the mother is also required. (Note: family status certificates, authorizations, solemn declarations, and other certificates can be issued through www.gov.gr).
- 5 The certificate may be collected ONLY: by the patient in person upon presentation of an identity card, by the parent who submitted the application, by an authorized person, by a courier service, by post, or by third parties with a proven legitimate legal interest.

Processing of Incoming & Outgoing Documents
The department operates an electronic document registration system. The database records the details of incoming and outgoing documents (sender, date, protocol number, subject, recipient). The protocol number is unique for each calendar year.
- The document, signed by its author and the appropriate supervisory hierarchy, is submitted to the Secretariat, where it is officially registered with the current date and time.
- Copies are made in a number corresponding to the recipients requiring action and those receiving the document for information.
- It is archived in the Secretariat's records. The messenger handles the distribution of the document.
- Documents intended for external distribution are sorted, placed in envelopes, and delivered by the messenger.
- Registered mail addressed personally to employees, as well as other personal correspondence, is not received by the Secretariat.
- The Secretariat certifies the authenticity of the signatures of the Hospital Governor, Deputy Governor, and Service Directors.
- Authors are requested to include the relevant reply protocol number so that incoming and outgoing documents can be cross-referenced.
